At the time of sign-up, the following information must be provided:
Photo ID for applicant with current address*
Proof of East Hartford residency (current utility bill in applicants name)
Proof of gross monthly household income (up to date budget sheet from State, if applicable)
Social security cards and birth certificates for all family members (long form for children)
Proof of dates of birth for all family/household members
Proof of guardianship/custody, if applicable
*Change of address forms are available 24 hours a day, 7 days a week in the lobby of the Public Safety Complex, 31 School St., E. Htfd. at no charge. Please make sure to have your current address on your photo ID at the time you apply.
APPLICATIONS WILL ONLY BE ACCEPTED ON THESE DATES AND TIMES.
NO APPLICATIONS WILL BE ACCEPTED OVER THE PHONE.
